HCPSS ACADEMIC ELIGIBILITY POLICY

In order to participate in try-outs for the Winter Athletic Season beginning November 15, student candidates must meet the HCPSS Academic Eligibility Policy with regards to their First Quarter Report Card. Students MUST have a Minimum 2.0 GPA and NO "E" Grades on their First Quarter Report Card in order to try-out for a Winter Athletic Team. Prior to trying out for any Winter sport, the following paperwork must be submitted:

• A physical dated after April 1, 2007; The physical is good for the entire 2007-2008 school year. Please make sure that the doctor’s office stamps the physical (either an address or signature stamp) when it is completed.

• Parent permission form; Please make sure to write the insurance company and policy number. Also, be sure to indicate whether or not the school may use pictures on the website of the school or school system. While it may seem obvious, please write the sport for which the student is trying out during that season only. A new form must be submitted for each season and sport.

• A copy of the student’s birth certificate (needed only once during each student’s high school career)

• Proof of residence within the Mt. Hebron school district, which may include:
o current utility bill (excluding cell phone bill)
o copy of lease or rental agreement
o copy of deed of trust
o property tax bill
